This seminar offers an in-depth review of proper documentation protocols from the physician’s order through evaluation, treatment and discharge. A detailed review of Medicare coverage criteria will be provided. General guidelines for therapy documentation will be examined as well as discipline-specific considerations for skilled documentation to support claims. A comprehensive review of elements known to trigger medical review including reason for referral, prior level of function, goals, coding to support the plan of care, prognosis and relevant history will be presented. Using multiple examples, therapists will learn how to be compliant with documentation and how to avoid common errors.
