Description: The ability to predict falls in persons with Parkinson (PwP) is critical. An article by Parashos et al in 2018 analyzed data from the National Parkinson Foundation of 3,798 participants found that 86% of PwP report no or rare falls and 12% reported at least monthly falls. The predictors of these falls include: Female gender, motor fluctuations, Levodopa and antidepressant treatments, prior deep brain stimulation surgery, reported worse quality of life, Hoehn & Yahr stages 2 or 3, worse semantic fluency, addition of the drug Amantadine, new diagnosis of cancer or osteoarthritis and increased emergency visits. Although this list can be useful when predicting falls in PwP, outcome tools that measure can more accurately predict falls desperately needed. A new clinical measures used to predict falls in PwP is the 3-Meter Backward Walk test (3-MBW test). This test was published in the Journal of Geriatric Physical Therapy in 2017 by Carter et al. The original research on the 3MBW test introduced the diagnostic accuracy of this novel clinical measure in 59 independent community dwelling older adults (mean age 71) without neurological conditions, found person who completed the 3MBW test in less than 3 secs were less likely to have reported a fall and persons who completed the test slower than 4.5 secs where more likely to have reported a fall in the past year. Dr. Carter is now investigating the diagnostic accuracy of the 3MBW test in PwP and has found that this tool is better at predicting retrospective falls then the Timed Up and Go (when set at 10 and 12 secs) as well as other fall predictive test.
