While social determinants of health significantly contribute to disparities that affect a wide range of quality of life risks and health outcomes, they can be challenging to conceptualize and address in clinical settings. By identifying, addressing, and continuously measuring the economic and social conditions that influence individual and group differences in health status, healthcare organizations can implement a program that ensures that front-line staff are prepared to effectively address the comprehensive needs of their patients, improving overall outcomes and reducing readmission rates. The purpose of the course is to introduce providers to the effects of social determinants of health and guide them in incorporating this information into assessment and intervention with individuals, groups, families and communities.
