Falls are a major concern for all aging services providers. Senior living communities are complex because staff and facility factors contribute to fall risk, and the residents have complex health and disability-related risks for falls. Evidence based fall prevention programs provided in senior living settings have the potential to be highly cost-effective compared with current practice. However, implementing quality fall prevention programs and reducing patient falls is difficult. It takes a team-based multidisciplinary approach to create a culture focused on preventing falls and improving client outcomes. This session will examine the components of client centered fall prevention collaboration between nursing, therapy and the multidisciplinary team. The learner will identify ways for the multidisciplinary team to work together to assess, choose and implement fall prevention programs that are compatible with your community. This session will also outline principles of effective documentation for nursing and therapy to support reimbursement for fall prevention efforts. The strategies addressed in this session will help learners go back to their communities and focus on continued quality improvement plans for fall prevention programs in their community.
